Hope offers hospice services to eligible patients. To ensure your comfort and well-being, our skilled caregivers focus on the whole you, not just your illness, to help you live each day to the fullest.
Initiating a referral is easy
Individuals who have a physician’s order can refer themselves or be referred by a:
- Family member
- Social worker or case manager
- Healthcare agency or hospital
- Nursing home or other care facility
They simply need to fill out the hospice Referral Form and submit it online, by mail or by fax.
Hope keeps the process simple and stress-free
Once we’ve received the referral form, our admissions counselors work quickly to get you the care you need. They will contact you or your family member to gather information and set up an initial assessment. They also contact the patient’s physician, who will need to provide the patient’s:
- History and physical records
- Physician order
Hope admissions counselors will then reach out to discharge planners, social workers or other referral sources to gather the information they need to promptly initiate care.
Eligibility requirements for hospice care
All of the services provided by Hope require a doctor’s referral. For Medicare reimbursement, end-of-life care (hospice) requires adults to have a prognosis of six months or less to live.
