Once we’ve received the referral form, our admissions counselors work quickly to get you the care you need. They will contact you or your family member to gather information and set up an initial assessment. They also contact the patient’s physician, who will need to provide the patient’s:
Hope admissions counselors will then reach out to discharge planners, social workers or other referral sources to gather the information they need to promptly initiate care.
All of the services provided by Hope require a doctor’s referral. For Medicare reimbursement, end-of-life care (hospice) requires adults to have a prognosis of six months or less to live.
