Medical Marketing and Customer Service
Representative
Full Time Position
Reports to Director of Marketing
Responsibilities:
-
Develops and implements a program of regular contact
and outreach to identify and secure new referral sources, build census,
and meet established goals.
-
Maintains on-going relationships with physicians; case
managers, discharge planners, and social workers in hospitals; and staff
in residential facilities and skilled nursing facilities to increase
census and preference for Hope Hospice services.

Determines primary decision makers and proactively
builds positive, solid, working relationships with members of the medical
community.
-
Works to build the brand of Hope Hospice as the
preferred hospice provider in our service area in order to achieve and
exceed Agency goals for growth and census management.

Maintains organized account information on each
client; tracks visits and results on Agency data base. -
Provides input and assists in organizing educational
and appreciation activities to enhance the Hope Hospice brand in the
community.
Requirements:
-
Proven record of accomplishment in achieving new
patient referrals and building relationships with referral sources.

Proven ability to generate and maintain relationships
with new and existing accounts.
-
Possess medical community knowledge and an
understanding of and commitment to the hospice philosophy.
-
Bachelor Degree; ideal candidate is an RN with medical
sales experience in the local market.

Highly motivated, energetic, and possess strong
interpersonal relationship skills.
-
Excellent spoken and written communication skills.
-
Able to work independently and perform the physical
activities of the job.

Computer literate; familiarity with electronic medical
documentation systems a plus.

Possess a valid California driver license, current
auto insurance, and a vehicle for field use.

On Call Registered Nurse
Full Time & Part Time On Call Nursing positions
Reports to the Director of Patient Services
Responsibilities:
-
Responsible for providing skilled nursing,
intervention, and supportive care to patients in the home
setting for evening, night, weekend, and holiday On Call duty in
accordance with the Hope Hospice Interdisciplinary Plan of Care
for each patient.
-
Must respond to medical or emotional crisis and/or
death of patient and make home visit to provide direct care as
appropriate.

Documents care in the Electronic Medical Record System
in accordance with agency guidelines and regulatory
requirements.
Requirements:
-
Possess a current license to practice as a Registered
Nurse in California.
-
Minimum of two years of varied work experience as a
professional nurse with preferably in either an acute, oncology,
Home Health and/or Hospice setting.

Per Diem Registered Nurse
Per Diem Nursing position
Reports to the Director of Patient Services
Responsibilities:
-
Responsible for the completion of assigned visits,
admission or liaison nursing functions based on staffing and patient/family
needs, and for assessment and care of patients in end of life care.

Responds to medical or emotional crisis and/or death
of patient and makes home visits to provide direct care as appropriate.

Maintains up to date patient records so that problems,
goals and interventions are accurately and clearly stated and changes are
reflected as they occur. Documentation is maintained in an electronic
medical record system.
Requirements:
-
Possess a current license to practice as a registered
nurse in California.

Minimum of two years of varied work experience in
either an acute, oncology, Home Health and/or Hospice setting.

Availability to work evenings, nights, and participate
in weekend and holiday rotation.

Social Worker
Reports to the Social Work Supervisor
Responsibilities:
-
Working under the direction of the Social Work &
Pastoral Care Supervisor and in collaboration with Interdisciplinary Team
members, counsels patients and families regarding psychosocial adjustment to
life threatening illness and long term disability.
-
Provides information, referral, and consultative
services to patients, families and staff; advises on appropriate utilization
of community resources.
-
Collaborates with other staff to provide a continuum
of care.
Qualifications:
-
MSW or BSW with appropriate experience and training
-
Shall have a minimum of two years experience in mental
health or other appropriate field
-
Hospice experience preferred
-
Able to assess and respond to the emotional needs of
the terminally ill patient and family in the home
-
Communicates clearly and effectively
-
Effectively delivers services within an
interdisciplinary group approach
-
Possess a valid California Driver’s License, valid
auto insurance, an acceptable driving record and an automobile for field use
